The effect of occupational therapy home programs on sensory processing and feeding problems in children with Down syndrome: a randomized controlled trial

Abstract

ObjectivesThis study aimed to evaluate the impact of an Occupational Therapy Home Program (OTHP) on sensory processing and feeding problems in children with Down syndrome. MethodsA randomized controlled trial was conducted with 38 children with Down syndrome (aged 3-10 years) in T & uuml;rkiye. Participants were assigned to an intervention group (n = 20) or a control group (n = 18). The intervention group received an 8-week OTHP incorporating oral sensory stimulation, structured chewing exercises, and food texture adaptation, while the control group received no intervention. At the end of 8 wk, the control group received a home-based intervention without post-assessment. Sensory processing was assessed using the Sensory Profile, and feeding problems were evaluated with the Screening Tool of Feeding Problems. ResultsResults indicated negative significant difference in the intervention group compared to the control group in oral and tactile sensory processing, food selectivity, feeding skill deficits, food refusal, and nutrition-related behaviors (p < 0.001). ConclusionsThe findings support the effectiveness of OTHP in improving feeding and sensory challenges in children with Down syndrome. The intervention particularly enhanced tactile and oral sensory modulation, reducing food-related behavioral problems and improving feeding patterns. Future research should focus on standardizing OTHP protocols for broader application and long-term effectiveness.
