Comparative Content Analysis of Lung Transplant-Specific and Disease-Specific Questionnaires: A Study Based on the International Classification of Functioning, Disability and Health

Abstract
Background: Monitoring functional status is essential for lung transplantation (LTx) success, yet the specific content of available instruments remains unclear. We compared the LTx-specific Valued Life Activities (LTVLA) questionnaire with established respiratory assesment tools using the International Classification of Functioning, Disability and Health (ICF) framework. Methods: Items from the LTVLA, London Chest Activity of Daily Living (LCADL), and St. George's Respiratory Questionnaire (SGRQ) were linked to ICF categories using standardized rules. Inter-rater agreement was analyzed via Kappa statistics. SF-36 linkage was derived from validated literature. Results: LTVLA items mapped to 47 categories, focusing on Activities and Participation (87.71%) and Environmental Factors (12.29%), with no Body Functions items. Conversely, SGRQ and LCADL showed a symptomatic focus, covering Body Functions at 39.50% and 44.12%, respectively. None of questionnaires addressed Body Structures. LTVLA uniquely captured environmental barriers. Inter-rater agreement was substantial to almost perfect (Kappa coefficient: 0.777–0.943). Conclusion: Our findings clarify the distinct clinical roles of these instruments. While the SGRQ and LCADL are effective for monitoring symptoms and respiratory impact, the LTVLA is uniquely suited to evaluate social participation and environmental barriers in lung transplant canditates and recipients.
