BoyaciogluDepartment of Dentomaxillofacial Radiology, Faculty of Dentistry, Hacetepe University, Ankara, TurkeyIntroduction: Gemination is a rare anomaly that occurs when a single tooth germ is divided into two in the mor-phodiversion phase. In rare cases, complete division along the crown and root may also occur. Generally, radiographs shows a large root canal with a single or partially separated pulp chamber. A supplemental tooth is a type of supernumer-ary tooth that is well formed to same a tooth and occurs distal to the lateral incisor. For its diagnosis, each tooth in the den-tal arch must be carefully counted and identified.Case report: In this case report, a combination of supple-mentary permanent lateral tooth and permanent central tooth with gemination in a male patient is presented. The patient admitted to our clinic for aesthetic complaints. The clinical and radiological examination revealed a gemination in tooth number 21 and a supplemental tooth distal to the right upper lateral tooth. The clinical appearance of tooth number 21 is in the form of an incisor tooth with a large crown, but divided into two. The radiological evaluation also confirmed the form of a double crowned tooth with an enlarged pulp chamber.Discussion: Twin tooth anomaly can be seen as fusion, gemi-nation and concrescence. The distinction between fusion and gemination can be made clinically only by counting the teeth.Conclusion: Dentists should be well informed of develop-mental dental anomalies and complications, and should be able to diagnose with clinical and radiological examination
