Autoimmune hepatitis/sclerosing cholangitis overlap syndrome with inflammatory bowel disease in a boy: Role of MR cholangiopancreatography in the diagnosis

Abstract

Primary sclerosing cholangitis (PSC) is a chronic cholestatic syndrome of unknown etiology. Diagnosis is based on clinical and laboratory data in conjunction with imaging of the biliary tree using mostly endoscopic retrograde cholangiopancreatography (ERCP) and less often percutaneous transhepatic cholangiography (PTC) that are both invasive techniques. We want to report an 11-year-old boy with autoimmune hepatitis (AIH) and sclerosing cholangitis (SC) overlap syndrome and inflammatory bowel disease (IBD) that was a rarely seen entity in childhood. Different from the reported cases, magnetic resonance cholangiopancreatography (MRCP) was used to confirm the clinical diagnosis of sclerosing cholangitis in this patient. As a non-invasive technique, magnetic resonance cholangiopancreatography may be the best preliminary method for evaluation of children. Due to high incidence of inflammatory bowel disease in overlap syndrome, especially in case of positive perinuclear anti-neutrophil cytoplasmic antibody level, early colonoscopy may be important. © 2004 Elsevier Ireland Ltd.
